How long does the referral process take?
Once I receive your referral form, I will contact you within 3 business days to discuss the next steps.
What documents do I need to prepare?
Please have your NDIS plan and any relevant therapy reports ready. This helps me understand the specific support needs of the participant.
Can I refer on behalf of someone else?
Yes, professionals and family members can complete the form. I just require the legal guardian's consent to proceed.
What if my NDIS plan is about to expire?
I can assist with plan reviews and ensure there is no gap in support. Mention your plan end-date in the form.
